The full logs in the members area of Workspace show periodic occurrences of the super_admin role being added to one or more users. The log indicates that the assignation was "Manual" and was performed by a user named "kadmin" and looks similar to this:

Upon any site update, (this includes the weekly stable update and any time we manually push a fix to your applications), the application defaults are reloaded.  The Super Admin user type is the default type that grants the super_admin role.  This type is updated when the defaults are reloaded; the role updating code runs and verifies that users with the Super Admin user type have the super_admin role. This is a routine maintenance process and is not assigning super_admin to anyone who has not been configured to have it.
